Two 250mg tablets the first day,  then 1 tablet per day for 4 days1
Chem Specs azithromycin
Quantities Z-Pak = Package of six 250mg tablets.   Tri-Pak = package three 500mg tablets.
Zmax = 1,000mg oral suspension, 2,000mg oral suspension.
Azithromycin availability: 250mg, 500mg, 600mg tablets. 200mg/5ml suspension
in 15, 22.5, 30ml bottle, 100mg/5ml suspension in 15ml bottle.

Class extended-spectrum macrolide
Action Blocks bacterial protein synthesis. (Binds ribosome 50s subunit.)
Usage Indicated in the treatment of sinusitis and skin structure infections.
Often used in treatment of inclusion conjunctivitis.
Warnings Use caution with known heart arrhythmias, angina, prior heart attack or chest pain.
Use caution with impaired hepatic function.
Contraindications Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic.
Pediatric use > 5 months
Pregnancy A retrospective study found association with increased incidence of miscarriage.2
Exposure during 1st trimester associated with increased risk of abortion and birth defects. (PDR)
For chlamydia the current CDC recommendation: One gram in a single dose
(2 tablets, 4 tablets, or suspension).   Resistance is endemic in many areas.
